District 202 will once again participate in the State of Illinois 5 Essentials survey.

The 5 Essentials survey provides a unique opportunity for students, teachers and parents to have a voice in improving their schools. 

Through February 15, 2019, students, teachers and parents will be asked to take a 25 -minute research-based survey to help identify strengths and weaknesses in their school’s learning environment. This survey is a great way for schools to reflect on the progress they have made creating and maintaining a culture and climate that helps all students and staff feel valued, secure and supported. 

*** Fourth through twelfth graders will take the survey during school***

PARENTS:

To receive parent data, a school must achieve a response rate of 20 percent from parents. 

• The total number of parents eligible to take the survey at a school will equal a school’s total student body.  
• Parents with more than one child at the same school may take the survey multiple times. 
• Parents with children enrolled in different schools may take the survey for each school. 
• Only one parent per household may participate in the survey. 

To complete the Parent Survey:

• Click on the link below and select “Parent Survey.”
• Select the necessary language to complete the survey (English or Spanish).
• Enter requested demographic information.
• Advance to survey questions.
• Answer all applicable questions.
• Click Submit Survey.

TEACHERS:  

Teachers will receive an invitation email with unique username and password to log-in to the survey and participate.   A teacher survey report will be generated if at least 50 percent of teachers respond to the survey.

To complete the Teacher Survey:

• Visit https://survey.5-essentials.org/illinois (this URL will be included in their invitation)
• Enters their unique username and passcode, which has also been provided via email.
• Answer all applicable questions.
• Click Submit Survey.

Individuals in the following positions, who also teach, are among those encouraged to participate:

• Self-contained and subject-specific classroom teachers.
• Instructional coaches and subject matter specialists.
• Teacher aides and paraprofessionals.
• Special education teachers working in a single classroom or across classrooms.
• Counselors, librarians, speech language pathologists and other staff members who teach students

STUDENTS:

Student participants will be required to log-in using their state student ID and birth date in order to complete the survey. It may be helpful to print and have these rosters handy when administering the survey, in case the state student IDs are not commonly known.  A student survey report will be generated if at least 50 percent of students respond to the survey.

To complete the Student Survey:

• Visit https://survey.5-essentials.org/illinois/ and select Student Survey.
• Enter state student ID and birthdate.
• Select the necessary language to complete the survey (English or Spanish).
• Answer all applicable questions.
• Click Submit Survey.
